Dear New Patient,

We thank you for choosing Conestoga Oral Surgery for your oral surgery needs. Our goal is to make your visit here as comfortable as possible.

CLICK HERE and complete our SECURE, HIPAA COMPLIANT online patient registration forms. By completing your paperwork at home, this allows our team to verify insurance eligibility and help complete the registration process before you arrive at your appointment. Uploading your DENTAL and MEDICAL INSURANCE CARDS as well as a Photo ID will also expedite the process. If you are unable to complete these online forms, please come to our office 15 minutes prior to your scheduled time to fill out your paperwork or select the printable option on our website and complete them before your arrival. We can also scan all dental and medical insurance cards and a photo ID upon your arrival. As a courtesy to you, we will be happy to submit insurance claims; however, we must have all insurance information.

Please arrive 15 minutes prior to your scheduled time to check in and have any necessary X-rays taken by our friendly staff. If you arrive later than your appointment time you may be asked to wait longer than usual or reschedule your appointment. If for any reason you are unable to keep your appointment, please call our office as soon as possible to inform us of this change.

A payment will be necessary at the time of your appointment even if insurance coverage is expected. Patients of minor age (17 years old and under) must be accompanied by a parent or legal guardian. There are NO EXCEPTIONS to this policy.
