At Conestoga Oral Surgery, our focus is to provide excellent, state-of-the-art oral surgery care to patients in Lancaster and throughout Central Pennsylvania. Our referral base is very important to us, and we want you, the referring dentist, to have easy access to the necessary tools to ensure your patients have a smooth and enjoyable experience in our office. You may click on the PDF file on this page to download and print our referral form. This form can then be given to your patient to bring with them to their appointment. You may also fax/mail the referral directly to our office with any pertinent radiographs.

In addition to the old, paper referral format, we also offer LOOP, a cloud-based HIPAA-compliant service to streamline the patient referral process. Many of our referring offices have already taken advantage of this FREE service, and we would be happy to assist you and your office staff with setup and staff training.
