At Conestoga Oral Surgery, our focus is to provide excellent, state-of-the-art oral surgery care to patients in the Central Pennsylvania region. Our referral base is very important to us, and we want you, the referring dentist, to have easy access to the necessary tools to ensure your patients have a smooth and enjoyable experience in our office. You may click on the PDF file to the right to download and print our referral form. This form can then be given to your patient to bring with them to their appointment. You may also fax/mail the referral directly to our office with any pertinent radiographs.



In addition to the old, paper referral format, we are also offering a new, cloud-based HIPAA compliant service to streamline the patient referral process at Conestoga. Many of our referrals have already taken advantage of this FREE service, and we would be happy to assist you and your office staff with setup and staff training.   • You no longer need to have your staff on hold with our office over the phone to ask questions or attempt to schedule a patient for an appointment
  • The paper referral can sometimes get lost or misplaced by the patient, and this can cause a breakdown in both efficiency and communication between both the dentist and oral surgeon
  • You as the referring dentist will be able to access your patients treatment timeline through LOOP to see where they are in their treatment process
  • Your patient has a much more streamlined process which reduces appointment time and increases overall patient experience
